Vaginismus is a joint defense mechanism of the mind and body. The brain, perceiving sexual intercourse as a threatening situation, attempts to protect the body by contracting the vaginal muscles. For effective treatment, it is necessary to identify the causes underlying this protective reflex. In vaginismus treatments, psychological and physiological causes are differentiated to apply personalized treatment protocols.

1. Psychological Causes (Most Common)

Approximately 90% of vaginismus cases in society are of psychological origin:

  • Lack of Sexual Education and Misinformation: Exaggerated stories of pain, tearing, and excessive bleeding on the first night.
  • Strict Moral Values and Feelings of Guilt: Upbringing where sexuality is coded as shameful, sinful, and dirty.
  • Past Trauma: Adverse childhood experiences, abuse, or traumatic gynecological examinations.
  • Performance Anxiety: Fear of not being able to please the partner or anxiety regarding sexual inadequacy.

2. Physiological Causes (Organic Reasons)

Although rare, certain physical disorders detected during gynecological examinations can trigger vaginismus:

  • Hymen Anomalies: A hymen that is thicker than normal, high-edged, septate (divided), or rigid mechanically blocks vaginal entry and triggers vaginismus due to pain.
  • Vaginal Infections and Dryness: Painful intercourse caused by chronic yeast infections or dryness due to estrogen deficiency, which leads to a habit of involuntary contraction.
  • Vulvar Vestibulitis Syndrome (VVS): A condition characterized by hypersensitivity and pain in the tissue at the vaginal opening.
